   I took option 4 for granted; it's "the best hospital in the universe" and    
   is run by nurses whose outfits and name reflect the Sisters of Plenitude,    
   even if they don't appear to be cats. With this in mind, I think the onus    
   is on T987654321 to provide any evidence at all that the hospital scene    
   *is* set in the 1930s.   
      
   5 is also plausible; River does seem to have a knack for time travel,    
   whether it's a black market Vortex Manipulator or the TARDIS herself (and,    
   I've just realised, it turns out that "I was taught by the best. Shame he    
   was busy that day" *wasn't* just a gag at the Doctor's expense).
