Alternate Kirk was born on a Federation vessel.
In our reality a ship carries a flag, making the ship belong to a certain
   nation.
But even if he was born on a Klingon ship or so, I would still call him an
Earth Human, and not alien (from a Terran perspective).

But what they called alien on TOS mostly looked very human and was very
   compatible.

   Other categories:

Close to human form (also mixable with humans in at least two cases):

   - The Vulcans
   - Andorians
   - Orions
   - Tellarites
   - The Klingons
   - Bele and Loki
   - The Apple folks (no, not Steve Jobs)

What would be really /totally/ alien within TOS?

   - The Salt Vampire
   - The Iron eating, people killing white cloud
   - The Horta
   - The Medusans
   - The various energy species / godlike entities (dubious maybe, at one point
they might have been more humanoid).
   - The original forms of Sylvia's people and the Kelvans.
   - The pancake parasites
   - The Tribbles
   - The Companion (sadly not Inara or Amy, gladly not anything from EFC).

Probably forgetting half of them right now.
