   You have to be interviewed by US and Canadian immigration officers,   
   who approve you if they agree you are a low-risk traveler.  In my   
   experience, that means you don't have a bad history and you have a   
   plausible reason for wanting the card.  I told them I had friends in   
   Toronto, and I drive up to YYZ to fly internationally.

   They'll revoke a NEXUS card if they think you're trying to sneak   
   through without properly declaring your stuff.  I gather this is a   
   chronic sore point in Point Roberts WA, where approximately everyone   
   has one.   
      
   Fun fact: NEXUS costs $50, while Global Entry, the program for US   
   airports, costs $100.  But if you have NEXUS, you can add on Global   
   Entry for free.
