If the Re was added before the editor then the user would be able to remove it. That is the thinking around why I did it that way. For the other stuff an early guess might be that its not adding Re if there is a space at the start of the subject line or something similar to that. If you can find a way to reproduce it let me know and I will get it fixed up...
