I'd start by running alacarte from a terminal - then, if there are errors,
you often have an opportunity to view them. Another good source for
trouble solving is to read the hidden file .xsession-errors - again from a
   terminal enter

    cat .xsession-errors | less

and read the output. Lastly, check your system for multiple instances of
alacarte running at once: in a terminal just enter

    top

   - once top is up pres the U key (but only that key) and then enter your
user ID. Then you'll see all your user's running processes, including any
instances of alacarte. If there are several alacarte listed you can kill
each by pressing the K key, type and enter the number of the process, and
chose 15 and enter that for each entry of alacarte.

Hope this gets you going in the right direction.
