   The bearing is worn and loose, but is re-tensioned by the change in engine   
   speed, creating a quiet mode of operation. I'd bet that it will come back   
   at a different, especially higher engine speed.

   Buy a new tensioner and replace it or have your mechanic do it for you.   
   That should fix the noise and shaking.
