I.  Seen while trying to get 3console connection:

1. typing "y", to accept the address, then switching to operate
code.   Must stay in test mode.

2. Using copy of the system software diskette, instead of the
original.  Use an original, they work the best.

3. Bad original system software diskette. Corrupt, try another.

4. Loading  3Console program from drive besides local a:.

5. Pressing return key after entering "y" to confirm Ethernet
address.

Can also be caused by problems with serial ports -on server or
on PEB.  Also, can be caused by if stray signals are being
returned to the server by serial devices (disconnect all serial
devices to verify this one).

II.  Seen while up and running, in thumbwheel 0:

1. Didn't put the toggle switch back to operate, after booting
up in thumbwheel 0.  When through installing, and boot up
in 0, put in operate mode for normal operation.  (in test
mode, if a signal comes in off the serial port, it will
cause the debugger active.)

2. Actual hardware malfunction.


III.  Seen elsewhere:

1. There is a thumbwheel setting (15) for debugger.  This is
the only time the debugger is suppose to be active (Toggle
switch set to test).

2. Seen as a symptom of interrupt 6 error.
