There was a problem with Tandy 4000 PC's running OS/2.  The floppy disk
controller that is on the motherboard is incapable of booting OS/2.  To
run OS/2, the following patch must be applied.  It is to the 12/15/87
release of DISK01.SYS which is 20133 bytes long.  Using debug type:

.br;Debug disk01.sys
.br;-e 4e72 c3
.br;-w
.br;-q

This will fix the problem with the floppy controller on the mother board.
The 4000 can also be fixed by using the controller card from Western Digital,
WD1003-WA2.
