 To "MM" I apologize for mistakenly posting at 
 misc.immigration.canada.  I intended to post at misc.invest.canada. 
  
 My main question is do the insurance companies or industry have an 
 ombudsman to resolve complaints?  From my previous problems with the 
 banks, I realized that customer service representatives are unable to 
 resolve disputes, so I had to escalate the problem way above.

 I worked for an actuarial and pension consulting firm for 5 years. If 
 you had asked the question in the appropriate forum I would 
 have given you a useful reply. But as it is, you are spamming. So
