 ADMINISTRATIVE ACTIONS - ALEXANDRIA 
  
 GLIDER AIRCRAFT ACCIDENT - 20 SEPTEMBER 2003 
  
  
  
 1.            Despite the tragic death of two pilots in the Tow plane 
 accident of Dec 1994, the cavalier and irresponsible flying conduct of 
 several Quinte Gliding Center pilots has not changed appreciably since. 
  
  
  
 2.         The Collateral Investigation, convened 20 Sept 2003 to 
 investigate the recent glider accident, reported that the accident was the 
 result of an attempt to conduct a prohibited flying maneuver.  It was also 
 revealed by the Collateral Board that senior pilots had flown similar 
 prohibited maneuvers, on infrequent occasions, at different locations, 
 mainly when deployed away from Mountain View.  This "culture" of 
 non-compliance to orders and regulations, and a complete and willful 
 disregard for established safe flying practices cannot and will not be 
 tolerated. 
  
  
  
 2.            Overall, the flight safety record of Quinte Gliding Center 
 pilots' has been less than admirable.  The off field landing in Alexandria, 
 September 2002 was yet another example of unauthorized flight operations. 
 Despite guarantees to the contrary from GC leadership at the time, it is 
 apparent from the most recent accident that the attitudes have not changed. 
 There appears to be a deep-routed misunderstanding of what is considered to 
 be approved conduct and acceptable flying maneuvers.  Strong leadership and 
 cooperation will be key to ensuring a return to professionalism and 
 responsible unit conduct.  As well, immediate corrective measures are 
 required. 
  
  
  
 3.         The following actions will be taken, while a further Unit 
 Disciplinary Investigation is being conducted: 
  
  
  
 a.               Both accident pilots are grounded and will not fly with 
 Central Region again. 
  
 b.               The Quinte Gliding Center Commander is removed from command 
 permanently. 
  
 c.               Quinte Gliding Center is hereby disbanded. 
  
 d.               Eastern Ontario Gliding Center (EOGC) will stand up to 
 deliver the Air Cadet Glider Familiarization program to the eastern portion 
 of "Central Region". 
  
 e.               EOGC will operate only from Mountain View (infrequently 
 from CFB Trenton or Picton and only as authorized by RC Air Ops O). 
  
 f.                All pilots flying for EOGC will be required to sign a 
 Statement of Undertaking confirming their, and the Gliding Center's, total 
 compliance with all orders and regulations; and 
  
 g.               Until permanent leadership positions are assigned, Major R 
 Sensabaugh is appointed as the interim Eastern Ontario Gliding Center 
 Commander. 
  
  
  
 4.         Total loyalty and cooperation is required to remove the stigma 
 attached to this area's flying operations. Any further violations of 
 governing regulations will continue to be dealt with in the most severe 
 manner. 
  
  
  
 5.         Our mission is to provide a safe flying environment and an 
 enjoyable flying experience for all staff and young air cadets.   The 
 maintenance of this is expected of each and everyone.
